Documenting care and reporting observations Duties and Responsibilities:In accordance with applicable state and federal law and regulation, the RN shall be responsible for, but not limited to: Care of patients through health promotion, maintenance, and restoration. Care toward prevention of infection, accident and injury. Assessing and reassessing the patient’s nursing care needs and providing nursing care services within the scope of RN license. The initial assessment shall be performed by RN. Monitoring the patient’s response to nursing care and coordination of services. Informing the physician(s) and other personnel of changes in the patient’s condition and needs. Teaching, supervising and counseling the patient family members and staff regarding nursing care and patient’s needs, including other related problems of the patient at home, as well as, assist the patient in learning appropriate self-care techniques. Only a registered nurse may initiate and delegate these functions, which may be delegated and reinforced by RN personnel: a. Taking vital signs, temperature, pulse, respiration and blood pressure b. Measuring height, weight, intake and output c. Monitoring blood glucose d. Collecting specimens, urine, stool, sputum and wound drainage e. Recording and reporting information as appropriate f. Veni-puncture for the sole purpose of collecting blood samples. Flushing the Heparin lock for the purpose of maintenance only g. Provide personal hygiene and activities of daily living h. Take telephone orders related to a home health patient t. Administer medication u. Other clinical intervention as needed within the scope of RN license Participating in in-service programs Nursing personnel shall document in the patient’s medical/health record: a. Necessary revisions to nursing care plan, the nursing care plan shall be initiated by a registered nurse. b. Clinical notes and progress notes, and c. A record of medications administered. After each administration of medication the following shall be documented by the nurse who administered the drug name and strength of drug, date and time of administration, dosage administered, method of administration, and signature of the licensed nurse who administered the drug. Medication shall be administered in accordance with all federal and state laws and regulatory requirements The registered nurse shall instruct and supervise the applicable Company staffing and shall give written instructions regarding the home health service to be provided. If registered nurse delegates selected tasks to the applicable Company staff the RN shall determine the degree of supervision to provide, based upon evaluation of the patient’s condition, the education skill and training of the applicable Company staffing which the task(s) is delegated and the nature of the tasks and activities being delegated. The RN shall delegate task(s) only to the applicable Company staff who meets the requirements as specified by the North Carolina Board of Nursing and the North Carolina Department of Health and Human Services or other authorized regulatory body, and who has demonstrated the knowledge skill and competency to perform the delegated task. The RN shall make supervisory visits to the patient’s home in accordance with the Agency’s policies and procedures. Maintaining patient confidentiality The person in this position must maintain good working relationship with Company staff and patients as well as a sympathetic attitude towards care of the sick. The person in this position must be sensitive to the needs and characteristics of the population serviced by Company. This employee must also attend a minimum of twelve (12) hours of in-service education each year.
